FORM was founded in 1997 by an environmentally conscious Myanmar doctor, an expat, who lives and works in London, England. It is registered as a charitable organisation in England and Wales. Majority of five trustees are also medical doctors who work in London and all trustees work as volunteers. FORM was also set up as a 501 (3) (1) organisation in Maryland, USA in 2001. It was also registered, according to Myanmar's law, as Non-Governmental Organisation (NGO) in Myanmar in 2000. The seven member board of directors in Yangon including highly experienced individuals in the field of forestry and environment manage the day-to-day operations in Myanmar.
